Enrollment Verification Letter Request Form

For questions about your enrollment verification letter, please reference the information below:
  • Timeline: Please allow up to 3 weeks for requests to be processed. (Note: they are usually processed sooner.)
  • Eligibility: Students/alumni must be actively enrolled or previously enrolled to obtain an enrollment verification letter.
  • Notice: You will receive a confirmation email from the Registrar's Office after you fill out this form, and another email once we process your request.
  • Questions: Please reach out to support@relay.edu.

Your Relay Student ID can be found on your Student ID and in emails/communication sent from your campus or Relay.
 
6. When do you want your enrollment verification letter? *This question is required.
Degree-conferred enrollment verification letters for graduates who have satisfied their program requirements on time will be available by the date of the graduation ceremony. 
7. Do you want a hard copy, soft copy, or both? *This question is required.

8. Should your enrollment letter include total completed hours of the Leadership program? (Note: We can only include this information if you completed the Leadership program)
