Directory Non-Disclosure

If you are a student who does not want any or all directory information to be disclosed without prior consent, please submit this form. Otherwise, directory information may be made available to any parties deemed to have a legitimate interest in the information according to The Family Educational Rights and Privacy Act (FERPA).  Disclosure preferences can be changed at any time by submitting a new form to the Registrar Office.
